Our client is seeking a compassionate Home Health Physical Therapy Assistant to join their extraordinary care team in Springfield, IL. The role involves collaborating to provide patient care plans, supporting patients in the area, and demonstrating skills in time management and technology use.

Key Responsibilities:

• Collaborate to provide management and delivery of patient care plans

• Support patients in Springfield, IL and surrounding areas

• Consistently meet critical deadlines for charting

• Self-manage time and schedule

Qualifications:

• Associate degree or higher

• 1+ years of experience in ANY of the following:

  • Electronic Medical Record (EMR)

  • Other EMR

  • TruChart EHR

• 1+ years of experience in ANY of the following:

  • Acute Care

  • Rehabilitative Setting

• Valid Physical Therapy Assistant certification/license in the state of Illinois (IL)

• Valid Regular Driver License (DL) certification/license
